Public Liability Insurance (Type your Policy No & Expiry - A copy will be required if your application is successful)

Food Business Registration (Type YES or NO - A copy will be required if your application is successful). All food vendors and anyone with pre packaged food for sale will be contacted by the Shire of Dardanup’s Environmental Health Officer
