Public Liability Insurance (Type your Policy No & Expiry - A copy will be required if your application is successful)

Food Business Registration (Type YES or NO - A copy will be required if your application is successful). All food vendors and anyone with pre packaged food for sale will be contacted by the Shire of Dardanup’s Environmental Health Officer

Do you plan to use a generator (must be whisper quiet.) If so, what size is the generator?

Please indicate if you will need a parking spot within close proximity of your stall. One parking spot peer stal would be considered. Please note that not all requests can be met.
